Who Is a Candidate for Under-Eye Filler?

Under-eye hollows often begin to develop in the early thirties as volume gradually decreases beneath the eyes. The depression may appear in the inner corner of the eye, along the lower eyelid, and sometimes extend into the tear trough between the cheek and the nose.

Please note: Hyaluronic acid fillers improve hollow under-eye circles but do not treat dark pigmentation.

  • Purple or bluish circles are usually caused by the underlying muscle showing through the thin eyelid skin. In these cases, micro-fat grafting may be a more suitable option.
  • Brown circles are typically caused by excess pigmentation and may respond better to depigmentation treatments such as chemical peels.

Consultation Before Treatment

During your consultation, I carefully examine the eye area to determine the most appropriate treatment plan. This includes evaluating whether the filler should target only the under-eye hollows or both the hollows and the tear trough.

If prominent under-eye bags are present along with volume loss, lower eyelid surgery (blepharoplasty) may provide a more suitable and longer-lasting solution.

Procedure

Before the treatment begins, we review the planned injection areas together. A topical anesthetic cream is applied and allowed to take effect for approximately 30–40 minutes.

The injection procedure itself usually takes 20–30 minutes.

Using a fine blunt-tip cannula, the filler is carefully placed beneath the lower eyelid, between the bone and the muscle. The product is injected deeply along the bone rather than into the superficial fat to minimize visibility beneath the delicate eyelid skin.

A lightly cross-linked hyaluronic acid filler with low water absorption is selected to reduce the risk of swelling. Small, precise amounts are injected gradually to restore volume while maintaining a smooth, natural appearance. The tear trough is also treated when necessary to achieve optimal facial harmony.

You can return to your normal daily activities immediately after treatment, as no social downtime is required.

Results are visible immediately and continue to improve over the following 2–3 days.

Because the eyelid area is delicate, mild bruising may occur more frequently than in other facial areas. Any bruising typically resolves within a few days and can usually be concealed with light makeup once appropriate.

For the first 24 hours, it is recommended to avoid:

  • Intense physical exercise
  • Saunas and excessive heat exposure

A follow-up appointment is scheduled 2–3 weeks after treatment to assess the results and perform any minor refinements if needed.
